Findings
Alleged Victim (AV) relies on the facility for his/her care. Between December 31, 2022, through February 19, 2023, AV has sustained eight (8) falls, some resulting in injury and one resulting in a fracture of two (2) vertebra to AV. The facility failed to provide AV with a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
