- On February 19, 2023, Alleged Perpetrator 2 (AP2) engaged in a heated conversation with Alleged Victim (AV) and made inappropriate verbal threats towards AV who is a resident at the facility with known behaviors. AP2 choose to continue to engage in a heated conversation with AV despite AV's visible distress. AP2 failed to de-escalate the situation, instead continued to speak to AV in a harsh tone causing AV to respond in a negative manner. AP2's actions is considered emotional abuse due to AP2 using intimidating and verbal assaults towards AV who is a resident at the facility while working in the capacity of caregiver. AP2's actions caused AV emotional harm which constitutes emotional abuse. The facility failed to provide AV a safe environment which is a violation of Oregon Administrative Rules. AP2 allegedly physically abused AV. An investigation inconclusively determined no physical abuse occurred.
