- Alleged Victim (AV) relies on the facility to provide a safe environment. Between November 2022 through February 2023, AV suffered multiple falls. On or about February 20, 2023, AV suffered a fall which led to an injury and subsequently being sent to the hospital. On or about February 17, 2023, AV suffered another fall in which AV also suffered an injury. Despite multiple falls, including recent ones, the facility failed to place any preventative measure to help reduce AV’s risk of falls, which puts AV in risk of harm. The facility failed to properly care plan around AV’s known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
