Findings
Alleged Victim (AV) relies on the facility to administer his/her medications. AV had two medication changes and the medication was delivered to the facility on or about February 9, 2023, but not given until February 17, 2023. Documentary evidence indicates that Alleged Perpetrator 2 (AP2) could not prove that he/she notated or made anyone aware that the new medication for AV had arrived on the evening of January 9, 2023. In addition, the medication in question meds #1 and #2 were found in the med room days after having been received, no notes or instructions were attached. AP2 failed to provide a safe medication administration system, which is neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
