Findings
According to an investigation, on or about February 20, 2023, Alleged Perpetrator 2 (AP2) administered AV a medication not prescribed to AV. AV experienced side effects due to the medication error, including pain, itching and a full body rash. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe medication management system which is a violation of Oregon Administrative Rules.
