Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have his/her incontinence brief changed once every hour and to apply barrier cream due to a rash in peri-area. On or about February 21, 2023, AV’s rash was found to have gotten worse and it was discovered that facility staff were not consistently following AV’s care plan, causing AV unreasonable discomfort. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
