Findings
The facility failed to follow Witness #4's (W4) care plan to have one on one (1:1) staff monitoring due to behaviors. On or about February 21, 2023, the facility was short staffed and not providing the 1:1 staff to W4, when W4 hit the Alleged Victim (AV) in the face, causing AV’s face to turn red and causing unreasonable discomfort. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
