Findings
On or about February 22, 2023, the facility failed to follow Witness #4’s (W4) care plan which states W4 needs 1:1 supervision due to behaviors. The failure resulted in W4 choking the Alleged Victim (AV) as AV was sleeping on a sofa in the facility common area, causing AV unreasonable discomfort. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
