Findings
According to the documentation, the Alleged Perpetrator (AP3) failed to protect the Alleged Victim (AV) from verbal abuse. The failure resulted in the AP3 yelling a derogatory term to the AV causing emotional harm, which is a violation of resident rights and is considered verbal abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
