Findings
Witness #1(W1) has a history of aggression with other residents. On or about March 6, 2023, W1 hit the Alleged Victim (AV) on the back and shoulder while AV was walking by. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
