Findings
The Alleged Victim (AV) was care planned to be escorted to and from the dining room for every meal. According to an investigation, on or about March 13, 2023, the AV fell while using his/her walker to ambulate on his/her own to the dining room for lunch. As a result of the fall, AV experienced unreasonable discomfort and bruising to his/her back and lower leg area. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
