Findings
On or about March 17, 2023, the Alleged Victim (AV) wandered into Witness #1's (W1) room, where W1 hit AV, causing him/her to fall to the floor and sustain a skin tear on his/her head. W1 has a history of aggression and staff are to redirect W1 from other residents and are to redirect residents away from W1. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
