Findings
The Alleged Victim (AV) has suffered previous falls. On or about March 19, 2023, AV suffered a fall after using the restroom, was transported to the hospital and diagnosed with a broken arm. The facility's failure to properly care plan for AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
