Findings
The facility and Alleged Perpetrator 2 (AP2) failed to follow Witness 1's care plan and provide appropriate supervision according to his/her known behaviors and altercations with other residents. The failure resulted in W1 engaging in a physical altercation with the Alleged Victim, causing unreasonable discomfort, which is a violation of resident rights is considered neglect of care and constitutes abuse.
