Findings
The Alleged Victim relies on the facility for his or her safety. On or about March 5, 2023, AV was found outside the locked facility near the building loading dock. AV has a history of eloping from the facility two (2) previous times. The facility does not know how AV got out of the secure environment. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
