Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have skin protectors on his/her arms. On or about March 24, 2023, Alleged Perpetrators #2-4(AP2-4), were assigned to care for the AV, and AV was found without the skin protectors on and a scratch on AVs elbow. An investigation revealed AP3 failed to follow the care plan, and AP2 and AP4 results were inconclusive. AP3s actions/inactions is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
