Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about April 3, 2023, Alleged Perpetrator 2 (AP2) was told to wait before transferring AV but went ahead and did it on his/her own. as a result, AP2 caused AV really bad pain and AV suffered a skin tear on his/her elbow. Later that day, AV’s hand appeared bruised and swollen. AP2 failed to provide AV with a safe environment, which is neglect of care and constitutes abuse. The facility failed to assure AV was provided with a safe environment, which is a violation of Oregon Administrative Rules.
