- On or about April 5, 2023, the Alleged Victim (AV), fell out of bed. The Alleged Perpetrator #2 (AP2), found AV on the floor, and instead of providing assistance to AV, AP2 placed a pillow under AV's head and a blanket on them and left AV on the floor. AV was on the floor for approximately 15-20 minutes until another staff member came back from lunch and called 911. AV was taken to the hospital due to a head hematoma. AP2 failed to provide services to AV, which is considered neglect of care and constitutes abuse. The facility failed to ensure care was completed which is a violation or Oregon Administrative rules.
