Findings
Alleged Victim (AV) relies on the facility to administer his/her medication. Alleged Perpetrator 2 (AP2), Alleged Perpetrator 3 (AP3), and Alleged Perpetrator (AP4) were all trained on medication administration. AP2, AP3, and AP4 have all stated to have administer the wrong dose of pain medication to AV on various dates, putting AV at risk of harm. AP2, AP3 and AP4 failed to provide a safe medication administration system, which is neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
