Witness #1 (W1) is care planned for inappropriate behaviors. On or about April 9, 2023, W1 was found in the Alleged Victim's (AV) room, kissing AV. AV was emotionally upset after the interaction. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
