Findings
The Alleged Victim (AV) relies on the facility to provide a safe medication administration system and oversight of care. According to an investigation, on or about April 1, 2023, AV was discharged from the hospital with new medication orders. The facility and Alleged Perpetrators 2, 3, and 5 (AP2, AP3, and AP5) failed to provide oversight of the medication administration system and gave AV incorrect medication on April 4 and April 5, 2023, resulting in AV suffering from unreasonable discomfort. This is a violation of resident rights, constitutes neglect which is abuse. The allegation that Alleged Perpetrators 4, 6 and 7 (AP4, AP6 and AP7) were investigated and determined to be not substantiated.
