Findings
The facility failed to follow the Alleged Victim's (AV) care plan to provide one on one supervision for behaviors. The failure resulted in AV wandering into Witness 1's (W1) room, aggravating W1, causing him/her to hit AV in the chest and kick him/her in the leg, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
