- The facility failed to develop a comprehensive care plan for the Alleged Victim (AV) to ensure facility care staff were properly trained when AV suffered a seizure. Based on facility documentation and interviews, AV has a long history of experiencing seizures due to his/her medical diagnosis. A care plan was completed for AV on the day of admission, which was April 13, 2023. However, the care plan was not shared with facility care staff. AV experienced a seizure on the morning of April 21, 2023. At the time of AV's seizure on April 21, 2023, facility staff had not been provided any training or given written instruction regarding how to recognize a seizure or how to ensure AV's safety during and after a seizure which caused AV unreasonable discomfort and put AV at risk of serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
