Findings
The facility failed to implement effective interventions and provide adequate supervision according to Witness 1's (W1) known behaviors and history of altercations. The failure resulted in W1 pushing the Alleged Victim, causing him/her to lose their balance and experience unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
