Findings
On or about April 30, 2023, the facility failed to administer Alleged Victim's (AV) medication as ordered. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2) allegedly failed to administer AV's medication as ordered. An investigation determined no abuse occurred by AP2.
