Findings
Alleged Victim’s (AV) has history of aggressive behaviors with staff. AV is serviced plan for staff to intervene when AV has increased agitation with redirection. Staff to offer snacks, walks, and engage AV in puzzles. Staff are to avoid using the word “no” and stand and talk with AV. On or about May 07, 2023, AV was turning on and off the water approximately six (6) times; each time Alleged Perpetrator #2 (AP2) would say “don’t turn on the water” and would shut the water off. AV threw a right and left fist striking AP2 in the jaw twice, AP2 responded with an open hand slap to AV face. Shortly after AP2 self-reported the incident. AP2's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility’s failure is a violation of Oregon Administrative Rules.
