Findings
On or about May 14, 2023, Alleged Victim (AV) left the locked memory care facility without assistance and without injury. Based on facility documentation and interviews, AV has a history of exit seeking with a high risk for elopement. The facility's failure to provide a safe environment for AV is a violation of Oregon Administrative Rules.
