Findings
The facility failed to provide a safe medication administration system. On or about May 18, 2023, the Alleged Victim (AV) was sent to the hospital due to not being able to follow cues, get up and walk independently, or open his/her eyes. According to an investigation, AV's change of condition was a result of AV not receiving his/her mental health medication for approximately eighteen days due to a medication discontinuation error. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
