- The Alleged Victim (AV) is care planned for use of a Hoyer during transfers. According to an investigation, on or about May 22, 2023, AV was being transferred from his/her chair to the toilet when the Hoyer mechanism hit AV in the forehead, resulting in AV’s head bleeding. The facility failed to provide a safe environment and to ensure facility staff were properly trained using a Hoyer for transfer, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that AP2 failed to a safe environment was investigated, and the determination was not substantiated.
