Findings
Alleged Victim (AV) relies on the facility for his/her care, AV is known to get in altercations with other residents. On or about March 4, Witness 1 (W1) slapped AV in the face and caused AV to have a small scratch on AV’s chin. The facility failed to provide a safe environment for AV known AV’s history of altercations, which is a violation of resident rights, is neglect of care and constitutes abuse.
