Findings
The facility failed to appropriately monitor Witness 1 (W1) related to his/her known behaviors. On or about May 20, 2023, W1 and Alleged Victim (AV) had a resident-to-resident altercation in which W1 squeezed AV's hand hard causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
