- Alleged Victim is known to be a high fall risk resident. Per the care plan, staff are to be always next to AV when AV is transferring and ambulating. On or about June 21, 2023, AV had an unwitnessed fall. Staff heard the fall and found AV on the floor in the dining area. The facility failed to follow the care plan around been next to AV to avoid AV from sustaining falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
