- Alleged Victim is known to be a high fall risk resident. Per the care plan, staff are to be always next to AV when AV is transferring and ambulating. On or about July 8, 2023, AV sustained a second fall within the last 30 days. Staff found AV in the floor in the common area. The facility failed to follow the care plan around been next to AV to avoid AV from sustaining falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
