- The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, number of falls with lack of appropriate interventions. According to an investigation, AV experienced multiple falls, between April 29, 2023, and May 22, 2023. On or about May 25, 2023, AV experienced another fall, which resulted in AV being sent to the hospital and diagnosed with a hip fracture. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
