Findings
According to an investigation, between May 11, 2023, and May 22, 2023, AV experienced unreasonable discomfort during Hoyer transfers, including bruising and pain. The facility failed to provide a safe environment and to ensure facility staff were properly trained using a Hoyer for transfer, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
