Findings
The Alleged Victim has a history of falls when his/her knees give out and is care planned as a high fall risk. The facility failed to appropriately care plan, relating to falls. According to an investigation, or about June 1, 2023, AV experienced a fall when his/her knee gave out, resulting in unreasonable discomfort, including two skin tears. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
