Findings: The facility failed to implement interventions or appropriately care plan for Witness #1’s (W1) known behaviors that include wandering. On or about June 3, 2023, W1 wandered into the Alleged Victim (AV)’s room and attempted to lay on top of them in bed. AV was startled and began yelling for help, causing AV emotional distress, and placing them at risk for harm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01142 $500.00 fine assessed
