- Progress Notes from May 24, 2023, outlines Alleged Victim (AV) is weak related to a prior hip fracture. Between May 24th through June 6, 2023, AV fell six (6) times in the memory care unit. As of May 30, 2023, there were no fall interventions implemented after a fourth (4th) fall. There were no new fall interventions implemented after AV’s fifth (5th) and sixth (6th) fall that occurred on or about June 3rd and 6th, 2023. As a result of the falls, AV sustained skin injuries. The facility failed to care plan appropriately and mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
