Findings
The facility failed to follow W1's care plan to provide one on one supervision due to altercations with other residents. The failure resulted in an unwitnessed physical altercation with the AV causing unreasonable discomfort and bruising, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
