Findings
According to an investigation, on or about, June 12, 2023, the Alleged Victim’s (AV) reported approximately $200-$300 missing from his/her wallet. AV did not have access to a lock box and/or locking drawer. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care which constitutes abuse.
