Findings
The facility failed to follow W1's care plan to monitor him/her when in common areas to provide interventions as needed for behaviors. The failure resulted in W1 striking AV in the back of the head while in the common area unsupervised, causing AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
