According to the documentation, the facility failed to properly care plan for Witness 1 (W1) for behaviors regarding sexual contact with other residents. The failure resulted in W1 touching the Alleged Victim (AV) inappropriately causing a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
