Findings
On or about May 19, 2023, the facility failed to administer Alleged Victim's (AV) medication as ordered. Based on facility documentation and interviews, AV was prescribed an antibiotic for 5 days that was not administered because it was not entered correctly on AV's MAR. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules.
