Findings
The Alleged Victim (AV) is a known fall risk. AV suffered falls in March and April 2023, with no new interventions put into place. On or about June 22, AV fell again and experienced hip pain. was transported to the emergency department and diagnosed with a hip fracture. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
