- The facility failed to properly plan care for the Alleged Victim's (AV) mental health which resulted AV attempting suicide on June 3, 2023 and again on June 25, 2023. Based on interviews and facility documentation, the facility failed to have a safety plan through mental health. The facility's failure to provide basic services necessary to maintain AV's health and safety is a violation of Oregon Administrative Rules.
