Findings
On approximately 3 separate occasions, the Alleged Victim (AV) failed to receive his/her medications as ordered. AV's medications were left on his/her bedside and staff did not watch AV take his/her medication. This action placed AV at risk for harm for not receiving medications as ordered. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
