Findings
On or about June 27, 2023, the facility failed to provide a safe medication administration system by failing to administer medications to Alleged Victim (AV) as ordered. Based on facility documentation and interviews, AV received the wrong dose of morphine. The facility's failure is a violation of Oregon Administrative Rules.
