Findings
According to the documentation, the facility failed to follow the care plan to ensure the Alleged Victim (AV) was receiving and consuming their health shakes. Between approximately May 01, 2023, and June 29, 2023, the AV lost approximately 8.9 pounds or approximately 7.6% of their body weight, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
