- The Alleged Victim (AV) is known to watch doors to try to exit the facility, and has eloped the facility prior. On or about July 11, 2023, AV exited the facility through an unlocked door. At approximately 6:15 am, staff thought they saw AV at the bus stop. When staff went to the bus stop, AV was not there. Staff drove to AV's spouses residence and found AV standing outside the door of the home. AV was uninjured, however, AV was placed at risk of harm. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
