Findings
The Alleged Victim (AV) is a known fall risk. On or about March 7, 2023, staff heard AV yelling for help. AV was found on the floor of his/her bathroom. AV had soreness to his/her back, neck and head. AV was taken to the hospital and diagnosed with a head injury. The fall risk section of AV's care plan is vague and inadequately addresses AV's fall risk. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
